This is part of our CKD management program, which offers a simpler, more patient-centric approach to help delay the onset of end-stage renal disease, reduce hospital admissions, and support people with treatment options.

Transform Diabetes Care

Approximately 34 million Americans have been diagnosed with diabetes, and managing this disease costs an average of more than $9,600 annually per patient. Despite that investment, 70 percent of patients are unable to control their A1C levels over the course of a year. Transform Diabetes Care, launched three years ago, is an insights-driven solution designed to help reduce the complexity of self-management, improve health outcomes for plan members, and prevent the onset of diabetes for those at risk of diagnosis. Since its launch, it has demonstrated positive clinical outcomes and results. In late 2020, we unveiled our next-generation Transform Diabetes Care program. This updated solution addresses 120 possible gaps in care and features interventions across more clinical categories, individualized care at the member level and localized member-level interventions – all for a reduced cost. We launched with two large Aetna Group Medicare clients and plan to roll it out to PBM and Aetna self-insured clients in 2021.

Managing chronic kidney disease

An estimated 37 million Americans live with chronic kidney disease (CKD), and nearly 700,000 have end stage renal disease. Dialysis alone costs Medicare approximately $120 billion annually. Despite the enormous cost, patient outcomes are poor and nearly one in six dialysis patients die in the first year after starting treatment. CVS Kidney Care is working to fundamentally transform treatment, improve results and reduce costs for both patients and payors. Our CKD management employs advanced analytics for early identification of kidney disease and includes targeted patient engagement and ongoing education to help slow its progression. Through our collaboration with Satellite Healthcare, we also provide state-of-the-art dialysis centers that offer both home and in-center dialysis options to meet each patient’s unique treatment needs. Our proprietary HemoCare at-home dialysis device – currently in clinical trials – extends these capabilities. By allowing for longer, more frequent treatments, it should further enhance patient outcomes.

Transform Oncology Care

Transform Oncology Care utilizes CVS Health’s local footprint and unique assets to improve patient outcomes and lower overall costs at every point of the cancer care journey. This comprehensive oncology management program is also anchored on a first-of-its-kind precision medicine strategy for payors. By using genomic testing results at the point of prescribing, we are helping patients start on the best treatment faster and also matching eligible patients to clinical trials. Among our assets, Transform Oncology Care includes CVS Health nurse-led care management – integrated with a payor’s existing programs – to create a more personalized experience for patients and their caregivers. Through our NovoLogix decision-support platform, we are also able to streamline the medications approval process. Aetna has adopted the program for fully insured commercial and Medicare populations and is being offered to Aetna’s self-insured population for January 1, 2022. We will continue to increase penetration as oncology practices return to normal. Transform Oncology Care is available to other health plans as well.
